Please complete this form and submit with payment. After the form has been reviewed by town staff, you will receive a confirmation email and signed copy of your application. This does not mean your application has been approved. A staff member will contact you regarding next steps in the application process. Please make arrangements to attend the scheduled hearing once the date is selected.
